What is Testosterone Replacement Therapy?

Testosterone is the primary male sex hormone and plays a critical role in muscle mass, bone density, fat distribution, red blood cell production, mood regulation, and sexual function. After age 30, levels naturally decline by approximately 1-2% per year.

Low testosterone (hypogonadism) affects an estimated 4-5 million men in the US, with symptoms including chronic fatigue, reduced libido, brain fog, mood disturbances, and loss of muscle mass. Despite its prevalence, fewer than 10% of men with low T receive treatment.

At Phlex Meds, our board-certified providers develop individualized TRT protocols based on comprehensive lab work, medical history, and ongoing monitoring. We use only FDA-approved testosterone formulations compounded by licensed 503B pharmacies.

Available medications

All medications are compounded by licensed 503B pharmacies using FDA-approved ingredients.

Injectable

Testosterone Cypionate

The gold standard for TRT. Deep intramuscular injection provides steady testosterone levels with weekly or biweekly dosing. Preferred by most patients for reliability and cost-effectiveness.

Dosing100-200mg/week (divided doses)
Expected Timeline2-4 weeks for initial effects
Injectable

Testosterone Enanthate

Nearly identical pharmacokinetics to Cypionate with a slightly different ester. Some patients report marginally less injection site discomfort. Same efficacy profile.

Dosing100-200mg/week (divided doses)
Expected Timeline2-4 weeks for initial effects
Oral · Estrogen Control

Anastrozole

A non-steroidal aromatase inhibitor prescribed alongside testosterone when labs indicate elevated estradiol. Prevents estrogen-related side effects including water retention and gynecomastia.

Dosing0.25-0.5mg 2-3x/week (as needed)
Expected TimelineEffect within 24-48 hours
Injectable · Fertility Preservation

HCG (Gonadorelin)

Maintains testicular function and fertility for men planning future children. Stimulates endogenous LH production to preserve spermatogenesis alongside exogenous testosterone.

Dosing500-1000 IU 2-3x/week
Expected TimelineOngoing alongside TRT

Important Safety Information — Testosterone Therapy

Warning

Testosterone therapy may increase the risk of polycythemia (elevated red blood cells), which can increase the risk of blood clots, stroke, and heart attack. Patients must complete regular lab work including CBC and hematocrit monitoring at prescribed intervals.

Common Side Effects

Acne, injection site soreness, increased hematocrit, changes in mood, testicular atrophy (without HCG), water retention, and changes in lipid profiles. Report difficulty breathing, chest pain, or leg swelling immediately.

Contraindications

History of prostate or breast cancer, untreated obstructive sleep apnea, uncontrolled heart failure, polycythemia vera, and women who are or may become pregnant.

Frequently asked questions

Do I need to get labs before starting?

Yes. We require a comprehensive blood panel including Total and Free Testosterone, CBC, CMP, Lipid Panel, PSA, and Estradiol. We can order these through our LabCorp partnership, or we can accept recent labs (<90 days) from your primary care provider.

Is testosterone therapy safe?

When properly prescribed and monitored by a board-certified provider, TRT is well-studied and generally safe. We require regular follow-up labs at 6 weeks, 12 weeks, and quarterly thereafter to monitor hematocrit, PSA, and other safety markers.

Will TRT affect my fertility?

Exogenous testosterone can suppress sperm production. If you're planning to have children, we can add Gonadorelin (HCG alternative) to your protocol to maintain fertility. Discuss this with your provider during consultation.

How are injections administered?

Most patients self-administer at home using small (27-30g) insulin-style syringes for subcutaneous injection. It's virtually painless— we provide detailed injection guides and video tutorials.

What if I want to stop?

You can cancel anytime with no penalty. Your provider will provide a tapering protocol if needed. Testosterone levels will gradually return to your baseline over 2-6 weeks.

Is this covered by insurance?

Phlex Meds operates as a cash-pay telemedicine practice. Our all-inclusive pricing is typically comparable to or less than insurance copays + specialist visit costs. We provide superbills upon request for potential HSA/FSA reimbursement.
